Thank you for using our website to obtain the registration forms needed to enroll your child. In order for your child to be registered, you will need to download, print and complete these forms and deliver them as soon as possible to the school your child will be attending. Many of the forms may be completed online before printing. Your cooperation is most appreciated.
Children reaching the age of five on or before January 1 will be eligible to enter kindergarten the preceding September.

Cannot be registered until State of Connecticut Health forms are completed, signed and stamped as indicated. - State of Connecticut Health Assessment Record must have Doctor's signature

District map and elementary school assignment by street address - You will need to present a Birth Certificate or Current Passport and
- Proof of Residency at time of registration

If you are considering enrolling your child in the New Canaan Public Schools but have not made a final decision, please download, print and complete the New Parent Information Form. This will help us plan for the appropriate number of teachers and classes needed next year.
