Health Forms

Forms must be filled in by a parent or guardian and completed by a physician before being submitted in hard copy to the school nurse.

The Health Assessment form is mandatory for Kindergarten, Grade 6 and Grade 10 physicals and MUST have the doctor's signature AND stamp.

Save the appropriate form (or the entire packet) on your computer, remembering where you saved it, then open from that location and fill in your information before printing it out to be completed by a physician.
